The
following MHSAA Medical Release form has been adopted by the MYWAY,
MMWA and Cardinal Attack W.C. If any skin condition is
suspect, you must have a physician use this form to release for any
wrestling activities. Absolutely no other form will be
accepted. To save yourself the headache, print this form and
take to the physician. Although your physician signs and
releases the wrestler, it can be overruled by a tournament director.
